Duo 2FA not working properly.

After setting up DUO on my Screen Connect, clearing my browser cache, then re-logging into my Screen Connect, I get the dialog from Screen Connect to enter my code, yet I get a Duo 2FA push at the same time. When I accept the PUSH nothing happens on Screen Connect, its only when I enter in my Duo  2FA code does it proceed to log me in. Clearly the duo function isn't working properly. For the OTP I have it set do 'duo:...my..email..address". 

It's also worth noting the Submit a Service Ticket server is broken, when I click on that link from within help in Screen Connect it takes me to a Server Error in '/' Application page.

Notes: The current error page you are seeing can be replaced by a custom error page by modifying the "defaultRedirect" attribute of the application's <customErrors> configuration tag to point to a custom error page URL.
